Inactivate Nursing License

An LPN, RN, or APRN who no longer wishes to hold active SD license(s) may inactive the license(s) by completing and submitting the Inactive Request Form and $10 fee, for the inactivation of each license, to the Board of Nursing office.

• Upon receipt, your license(s) status will be changed to inactive.
• You will no longer receive license renewal notices.
• You may reactive the license(s) at any time by completing an application for Reactivation.
